    I rarely get surprised and angry when I read an accident report but this one goes beyond almost anything else I have seen. In this video we will get to witness two MAJOR incidents happen to the same crew during the same day. @Aaron Lowe First I don't want to sound intolerant or anything like that, but in the Islamic culture is a hard thing to question authority figures! This is a known problem pointed, for example, in studies about military operations when Islamic troops have better equipment and better training but still loose, and the problem is the lack of a literally "possibility" to question a ranking office or act with autonomy to use an advantage without an authorization to do so. This occur in western cultures too, like, for example, in the Tenerife Accident, when the second in command on the KLM appears to know that they don't have clearance to run but don't act accordingly, but in this regard is easier to train a "western" first officer to question a captain than a middle eastern. Don't know if this is changing, in aviation specially, due to the fact that CRM is a important known safety net, but probably is a hard think to teach. Oh, by the way this cultural problem to implement a good CRM is present in every culture that authority figures have a position of superiority so strong to others that the huge gap of power don't stimulate questioning decisions, be the authority a father, be a military commander. And isn't a exclusivity of the Islamic culture, even today.

love your vids but as an editor myself, and filmmaker, I might make your beats a bit quicker between your shotlist. Again love your work, maybe a teleprompter and stick to the script? One can "trim the fat " so to speak simply by taking out some of the dialogue not necessary to convey your story, so you keep continuity but don't lose viewers via lag, in this day and age of short almost montage style editing I see everywhere, just beat, beat, beat, beat, beat, out... Anyways But keep shining and thank you for your expertise you really do have great content!

Seems to me they was totally trying to fly VFR in IFR condition. Like why not set 400 feet and descend 1000 feet per min or 500 ft per min. Use the flight systems if your going to go rogue like that. U don't go rogue and hand fly, especially over water. It's difficult to judge distance over water as demonstrated by these two. Also they obviously was not even reading there instruments to see there altitude. Along with, never go VNAV ( vertical navigation) and exceed 800 per minute if your trying to do your own descent angle from 1500 feet. I mean these guys broke not only company policy, EASA policy but fundamental flying policy's and rules. Like: 1. Not reading altitude on screen 2. Exceeding vertical speed parameters when so low to the ground. 3. Turning off systems that should not be turned off. Again if your gonna go rogue, please keep your basic flying skills in the cockpit! Ok and you should not go Rogue! Don't do it with people in plane.. passengers did not sign up for that. Do it in your own plane and no passengers!

Perhaps they were always doing it this way at Hoceima, because of local weather conditions. The captain fixated on descending below the clouds and this time forgot about the altimeter. Considering no one was hurt and ATR behaved like a flying boat, I believe they could take this occasion, snorkel a bit around, watch some fishes, and then blow the tanks, emerge on the beach and drive those 2km to airport. :D

I think culture also plaid a small part in this. the Captain seemed to have a big ego and is usually someone older than the first officer. In our cultures (Asian) it is highly frowned appon to talk back to a elder person, which can turn deadly, like we saw here. So the first officer might have been scared to respond or give callouts/warnings to this specific pilot, in the fear of loosing his job maybe. Just my anology.

Some may remember the Korean 777 that struck a landing system structure in San Fran which caused several Chinese youngsters to fall out onto the runway still in their seats. The first officer sat and watched the captain allow the plane to drop something like 20 or 30 knots below required landing speed. When he finally suggested more power, it was too late. Then there was the French Air flight 220 I believe that stalled at 35,000 feet and pancaked into the sea when the pitot tubes failed. The pilots then proceeded to crash the damn thing. The two pilots also fought over the controls as one insisted on climbing which kept them in stall condition. All they had to do was push the nose down but they waited much to late.

He shouldn't be accepting those commands from the capitan, but he did the right thing on the edge of when he should do it. Who knows how many times such dangerous landings have worked before, standards on a regional airline in a 2nd world country are questionable. also: If the weather is so bad that nobody can land safely, why isnt the airport closed? I have this theory, and mi d you I’m sure there is already a term for it...but the theory is that excessive experience creates excessive confidence in ones own ability. A sort of expertise bias. Like if you make the same approach a million times in good conditions you can “do the approach blind” if that makes sense. Seems to be the case here in my opinion. Anyone know if there is any studies on this in aviation ?

Senior training Captain??? ... what a JERK to not only put everyone in jeopardy but also to give such a bad example to the F/O, put F/O in such an awkward situation and disabling systems and repeatedly going below minimums and then lying about a bird strike and pushing the yoke forward when the F/O is pulling back to do a go around and then botching the go around procedure as well ... WOW ... ask yourself if you would want your family flying in the back of their airplane. It is difficult for a new F/O to stand up to a senior captain ... this company needs to do mandatory CRM and F/O's need to be given the backing that their concerns will be treated properly and backed by the company. They absolutely should be fired at minimum!

Seems like there should be some level of criminal penalty at least on the Captain. Willful disregard of human life, reckless endangerment, etc. This isn't a simple mistake, this is a wilful act to disregard almost every rule in the book. As a student GA pilot and a principal mechanical design engineer I am fascinating by this subject. Additionally both my late father and stepfather worked at Structures Lab RAE Farnborough. My father was specifically an aerodynamics engineer and worked on the Comet crashes investigation and flew as a flight observer as he was, in addition to his degree in aeronautical engineering, a qualified pilot. He also calculated the flight path that the Vickers Valiant needed to take in order to outrun the shockwave caused by the atom bombs it dropped on the Christmas Island atoll, a VERY close run thing, my mother didn’t find out until he returned home as he had flown in the aircraft during the bomb drops! I was 2 at the time! Keep up the great work.

In general aviation, are step down RNav approaches still used? I haven't flown IFR since before the full availability of GPS approaches. My father, a retired airline and military pilot flew as the safety pilot as I flew the final hours under the hood required for my instrument rating. We were making a step down approach to Tucumcari NM. Flat terrain desert. Amazingly, we had a pretty steady ~40 kt head wind right down the runway, and it wasn't bumpy, so my ground speed was obviously low. It's VFR weather. So I do the step down to my final decision altitude which was probably 500 feet. So I was to precede until the proper DME and then do a missed approach. As soon as I was stable at the decision altitude, my father said to take off my hood. We had such a slow ground speed,

Sadly pilots disregard standard procedures regularly. If no crash, near miss or something happens that ATC has to report no one knows. Pulling breakers is just one, but blowing through minimums without a go around is insane. Maybe I missed it or the the airport or their ATR couldn’t but the video didn’t mention why they didn’t go ILS.

Bird strike??? On what grounds?? The captains license should get revoked. The first officer is the only reason why everyone was stil alive and yet the captain has forced the plane lower and then released the control. He was either high, blind or about to have a seizure(adrenaline due to the event may have prevented it)...

For a non-pilot, it does make sense to go to a low altitude (below the clouds, but sufficiently above the water of course) well before the airport so that you can actually see the runway in the distance and still have time to go around. Why is this not done? Too dangerous? And why not use an ILS approach? I see planes here landing in complete fog using their instruments...

Being an aviator myself for 12 years I've watched a lot of aircrash investigation and one thing I've noticed is that there were unacceptable high amount of incidents /crashes either caused by highly experienced captains who ignored their copilots or due to intimidated copilots who stayed silent because of abusive captains. In my unfortunately very short career as a 737 Copilot I had very rare cases where the captain wasn't a really good at CRM but when I had one and regarding all the incidents I mentioned above I wonder how a captain can develop such personality because they all were first officers once, too. And they should know best what (especially new) first officers go through.
