We don’t substitute brackets, they have alternate part number brackets but these are the same design by approved vendors or a bracket upgrade that has been approved. The person or persons giving approval for this unapproved part is most guilty it’s a substandard part obviously

Imagine how many undiscovered maintenance parts incompatibility are mounted right now as i write this into in service planes that will fail in the near future that we don't know yet about and all those that will not lead to fatal outcomes.

The problem is that overly computerized plane cockpits like Airbuses means a computer programmer would have done a better job than pilot who are trained for the aerodynamic physics of flight and manual flying of a plane. These are not the same set of skills at all. A pilot must have excellent coordination, cool nerves in all situations and brain sensory response while a programmer knows how to interact quickly with a computer. I think the captain Pichet did an excellent job at gliding the plane but failed the computer side of his job and that is the partly a plane design problem. Too much computerization for the human brain is a risk in itself. The computerization should not be excessive that it exceeds the intuitive understanding of a human brain. And in the case of the Airbus that is a design fail. By the way low oil temp and high fuel pressure is of course indicative of a leak even a race car pilot knows that how can they miss that. The fuel is a coolant, when it leak the passing of new fuel cools the engine and the oil, and the high pressure is due to the fuel pump struggling to regulate fuel pressure because of the leak. I am sure many people knows this. I guess not the pilots.

It's so typical that the airline blamed the pilots for mismanaging the fuel situation. They prioritize profits, so safety takes a back seat. It's disgusting. Plus, there was apparently no precedent for this circumstance, and no training either. The supervisor who authorized the retrofit of the older part should have been blamed, or at least the maintenance culture that supported his decision.

The greatest mistake was from the maintenance manager, that probably due to corporate culture, insisted the use of the incorrect part leading to the failure. Any pilot error was due to something outside of their training and in sufficient ability to determine what was the actual cause of the problem. While the instructions indicated not to open the cross valve if a fuel leak is evident, that could not be confirmed. However when seeing a continued drop in fuel, the valve should have been closed.

to reduce the flight speed it was necessary to lower the landing gear at a distance from the landing runway and begin zigzaging movements before approaching the runway. I learned this on Flight simulator. I don't understand why the flight captain did not use these two procedures.

anyone else feel the captain jumped to assuming the fuel leak was a computer error with zero supporting evidence worryingly quickly? it reminds me of the stories the series has covered involving Get-there-itus-his internal logic chain was obsessed with reaching the destination and rejecting conflicting information that suggested there was a problem. if there's no real problem, they can keep going, so it has to be a computer glitch,no,no, it CANT be real, so he can ignore it- a good chunk of his brain seems to have been completely shut down until the tanks actually running dry jot him awake....... it just looks worse and worse for his basic competence the longer the situation goes on- his first response to the flameout was almost literally "this cant be happening!" after a long period of his plane all but screaming "im running out of fuel!"

Everyone saying "he should have done this or he should have done that," ought to sit down with an Airbus manual and see if they can digest all the complex technical systems required to fly and land such a plane. A pilot of one of these things has to know every part, every switch, and every alarm and know what each does. It's insane when you think about it. The planes are supposed to fly themselves. That he brought down the plane with all passengers still intact attests to his skill as a pilot. He clearly states nothing like that had happened to him. It was probably such a shock that he couldn't troubleshoot it.

Switching off of crossfeed should have been an improvised approach when fuel flow rate was higher than avg consumption. Airbus having such electronic feedback of a vast net of sensors would have given good idea of situation in time. Another option could be to throttle down eng 2 to verify reduced fuel consumption vs mss flow rate. But it seems the checklist definitely got the pilots puzzled at some point

I met this Captain in person ... He is quite a down to earth person ! and seriously incredible what he manage to do. Also dont get what ppl a judging from ... the first warning was OIl pressure ... not fuel like many said and nothing was showing and they follow the procedure ... did he make a mistake of not closing it ? yes he did but still manage to do the impossible of getting it safely Also this is base on what they could manage to get ... everything is sealed ... also they dont Hide like Airbus who just blame Both pilot and proceed to change the book ... Really ticks me off when profit comes first safety second, Yes a idle plane is not making money but fitting incorrect parts has been a issue many of times and sadly ended badly. Aircraft maintenance & safety should be number one priority and not under so much pressure because some man in a suit isn't making enough money for their liking.
